Step 1: Collect the following information:

•       Email address
•       Organization
•       Department
•       Title
•       DOH Job Title
•       Licensure

Step 2:  Click HERE to create an account

Step 3:  Fill in the required fields that you have collected in step 1

Step 4:  Select “Register”
