Who We Are
Care Compass Network (CCN) is a not-for-profit, community organization that supports the transformation of health care delivery through the development and convening of strong and resilient partnerships, the coordination of care, and the reduction of health care expenses.
CCN convenes and collaborates with over 165 partner organizations, which include hospital systems, social care/ human services providers, nursing homes, behavioral health and substance use disorder programs, social services agencies, and higher education systems.
